Job Description

The Admission Coordinator is responsible for supporting the daily functions/operations of the admissions office. Will be responsible for assisting patients transitioning to the facility from the hospital or community. This position also reviews medical, financial and other records with appropriate department heads to ensure guest/resident is appropriate for the facility.

Responsibilities:

  • Receives resident referrals via fax, phone call, or internet and follows Ciena’s Admissions process.
  • Completes bedhold calls on all hospitalized guests
  • Manage 30-day Medicare postcard mailings and maintain a database of appropriate recipients
  • Conducts facility tours for potential residents and families; ensures that information is available for others that conduct tours.
  • Conducts admission screening of potential patients; determines level of care, services required, equipment needs and insurance coverage, as indicated.
  • Authorizes acceptance of patients and coordinates admissions with appropriate staff.
  • Ensures level of care approval obtained and/or OBRA screen (PASSAR, MI/MR) when indicated.
  • Obtains prior approval authorization number from payer on a per case basis when applicable.
  • Helps coordinate services needed for the resident with clinical staff prior to admission (i.e. respiratory equipment, specialized bed, etc.)
  • Welcomes resident and family upon admission.
  • Ensures receipt of all necessary documentation from the referring institution.
  • Coordinates room assignment designation with Nursing Department
  • Ensures timely delivery of daily census reports and monthly month-end reports to Administrator and corporate staff using SNF INFocus.
  • Documents and tracks pre-admission referral activity on the PCC CRM

Education and/or Experience:

  • High School Diploma required.
  • 4 year degree preferred
  • At least one (1) year experience in healthcare, sales/marketing/insurance, or commensurate
  • education preferred.
  • Experience with managed care/insurance preferred.
  • Familiar with medical terminology.
  • Ability to use technology including AI driven software
