Admission Nurse – RN | Full-Time

City View Post Acute is seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our team as an Admission Nurse. This is a full-time opportunity for a nurse who enjoys coordinating admissions and ensuring residents experience a smooth, safe, and welcoming transition into skilled nursing care.

Pay Rate: $50 -$60.00 per hour

Schedule: Full-Time

Position Overview

The Admission Nurse plays an important role in welcoming new residents to City View Post Acute. This position is responsible for coordinating and completing nursing admissions, conducting thorough assessments, reviewing clinical information, and helping ensure each resident's care needs are communicated effectively to the interdisciplinary team.

The ideal candidate is clinically knowledgeable, detail-oriented, organized, and able to communicate effectively with residents, families, physicians, referral sources, and healthcare professionals.

Key Responsibilities

  • Coordinate and complete nursing admissions for new residents.
  • Review hospital records, referral documentation, diagnoses, medications, and physician orders prior to admission.
  • Complete comprehensive nursing assessments and identify residents' clinical needs.
  • Review and reconcile medications and relevant medical information.
  • Communicate resident needs, assessments, and clinical findings to the interdisciplinary care team.
  • Collaborate with physicians, nursing staff, case managers, therapists, and other healthcare professionals.
  • Ensure admission documentation is accurate, complete, and completed in a timely manner.
  • Assist with developing and implementing individualized care plans.
  • Educate residents and families regarding facility services, care plans, and expectations.
  • Monitor residents throughout the admission process and promptly communicate changes or concerns.
  • Ensure compliance with facility policies, state and federal regulations, and professional nursing standards.
  • Protect resident privacy and maintain confidentiality in accordance with HIPAA.
  • Perform other duties as assigned.

Qualifications

  • Current and valid California Registered Nurse (RN) license required.
  • Previous skilled nursing or post-acute care experience preferred.
  • Prior admission, assessment, or clinical coordination experience is a plus.
  • Strong clinical assessment, communication, and organizational skills.
  • Excellent attention to detail and ability to manage multiple priorities.
  • Ability to work effectively with residents, families, referral sources, and interdisciplinary team members.
  • Strong knowledge of nursing documentation and applicable healthcare regulations.
  • Compassionate, professional, and resident-focused approach.
