Job Description

Assesses current and potential residents’ for placement throughout the village.

Responsibilities

1. Refer residents to appropriate health care services.

2. Investigate and direct patient inquiries or complaints to appropriate medical staff members and follow up to ensure satisfactory resolution.

3. Explain policies, procedures, or services to patients using medical or administrative knowledge.

4. Provide consultation or training to volunteers or staff on topics such as guest relations, patients' rights, and medical issues.

5. Collect and report data on topics such as patient encounters and inter-institutional problems, making recommendations for change when appropriate.

6. Identify and share research, recommendations, or other information regarding risk management.

7. Reviews, assesses and approves Health Care Center admissions. Contact resident insurance companies for approvals.

8. Attends and participates in resident assessments for transfers and new admissions

9. Communicate with discharge planners for daily census updates.

10. Assist in obtaining paperwork for Health Care Center utilizing Allscripts/ECIN programs to obtain hospital referrals.

11. Visit current residents admitted to nearby hospitals on a regular scheduled ongoing basis (weekly if not more). Conduct visit with a Marketing representative or discuss strategy prior to visit.

12. Participate in weekly clinical management meeting, daily Health Care Center meeting,and Marketing meetings.

13. Coordinate with instructors of clinical rotations for students in nursing programs/CNA training courses from various colleges/institutions

14. Development and implementation of incident/accident program and is responsible for monitoring and involving medical personnel to achieve measurable improvements in program

15. Maintain and communicate by e-mail daily and/or weekly the long term waiting list and Medicare.

16. Develops and maintains a relationship within the organization to include all areas. Encourage teamwork between all areas of marketing, nursing and assisted living.

17. Attend and participate in all appropriate meetings and training opportunities in order to keep informed on current information and skill development.

o Federal, state and local regulations.

Qualifications

Education:

Graduate from accredited school of nursing

Experience:

Six months to one year geriatric experience preferred

Computer Skills:

Computer literate in word processing and data entry

Certificates & Licenses:

Current RN or LPN licensure from the State of Michigan

Valid CPR certification

Other Requirements:

Must have working knowledge of the LoC determination forms

Knowledge of Medicare denial process, forms, Medicare Part "A" days ,and certification.

VEH: Availability to perform On Call/Rotational responsibility as weekend facility supervisor
