Job Description

Location: Wooster, Ohio
Schedule: Full-Time | 9:00 AM–9:00 PM
Weekend Requirement: Every other weekend rotation
Holiday Requirement: Holiday rotation required

We are seeking a compassionate and experienced Registered Nurse (RN) to join our hospice team in an admissions and care coordination role. The ideal candidate will have experience conducting comprehensive patient assessments, coordinating care, educating patients and families, and supporting patients and loved ones throughout the hospice journey.

This position provides care across hospitals, long-term care facilities, assisted living communities, and home settings The RN will work closely with physicians, facility staff, hospital teams, patients, families, and interdisciplinary team members to facilitate timely hospice admissions, effective care transitions, and high-quality end-of-life care.

The successful candidate will demonstrate strong clinical judgment, excellent communication skills, compassion, professionalism, and a commitment to patient- and family-centered care.

Key Responsibilities

  • Conduct comprehensive patient and family assessments addressing physical, emotional, psychosocial, and spiritual needs.
  • Complete hospice assessments in hospitals, long-term care facilities, assisted living communities, and home settings.
  • Educate patients and families about hospice services, eligibility, available resources, and the admissions process.
  • Support patients and families in making informed decisions regarding hospice care.
  • Collaborate with physicians and interdisciplinary team members to develop and initiate individualized plans of care based on each patient's goals, preferences, and clinical needs.
  • Communicate clinical findings, patient status, and assessment information to hospice physicians to support eligibility determination and certification.
  • Provide ongoing patient assessments, symptom management, and care coordination for patients in contracted facilities.
  • Facilitate smooth transitions between care settings while promoting continuity and quality of care.
  • Collaborate with physicians, hospital personnel, facility staff, and other healthcare professionals to support timely discharge planning.
  • Assist with departmental needs and organizational initiatives as assigned.
  • Participate in quality improvement initiatives focused on patient outcomes, patient and family experiences, and service excellence.
  • Mentor and orient newly hired staff while promoting teamwork, professional development, and positive working relationships.
  • Maintain accurate and timely documentation of patient care and services.

Qualifications

  • Graduate of an accredited nursing program.
  • Current Registered Nurse (RN) license in the State of Ohio with no current board actions.
  • Two years of acute care nursing experience preferred.
  • Certified Hospice and Palliative Nurse (CHPN) certification preferred.
  • CHPN certification preparation and support will be available to Ohio's Hospice employees who wish to pursue certification.
  • Current CPR certification.
  • Computer skills sufficient to accurately document patient care and services.
  • Valid driver's license with an acceptable driving history.
  • Ability to travel as needed and safely navigate varying weather and road conditions.
  • Current automobile insurance meeting state minimum requirements.
  • Ability to successfully complete a background investigation and 10-panel drug screening.
