About the CAP Program

CAP provides Intensive supportive housing for adults with severe and persistent mental illness. Clients receive medically necessary on-site services in a clustered apartment setting because of a history of failure in multiple community settings and/or the presence on an ongoing risk of harm to self or others which would otherwise require long term psychiatric hospitalization.

What You’ll Do

  • Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
  • Facilitate each client’s achievement of long range outcomes of symptom management.
  • Engage difficult to serve clients by investigating all potential resources in order to gain access and make connections.
  • Coordinate medication-related care between clients, ReDiscover providers, outside medical providers, pharmacies, Housing staff, and the Housing Nurse to ensure continuity of care and timely communication regarding medication needs, changes, and concerns.
  • Maintain accurate medication records and MARs, including obtaining updated medication lists and orders, identifying and resolving discrepancies, and ensuring documentation reflects clients’ current prescribed medications.
  • Coordinate medication delivery and receipt across Housing locations, including being available for pharmacy drop-offs, picking up medications when necessary, checking medications in according to established procedures, and collaborating with the Housing Nurse and treatment team regarding any concerns or follow-up needs.
  • Assist clients with transportation barriers as identified in their treatment plan. This includes transporting clients in the employee's personal vehicle to various appointments and area resources.
  • Collect and maintain information about clients and document interactions within ReDiscover’s designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements of the Position

  • 4 year degree or 4 years of equivalent experience in lieu of degree.
  • A valid automobile driver’s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work hours which includes transporting clients in personal vehicle.
  • At least two (2) years clinical experience in a direct service provision to children and adolescents in a mental health setting preferred.
  • Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral and interpersonal communication skills.
  • Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as the use of email.
