Addiction & Mental Health Worker, Housing - Exeter
Direct Service - (OPSEU)
Full-Time, Contract (Ending: Approximately 18 months)
Exeter, Ontario (South Huron Zone)

The Addiction and Mental Health Worker will perform duties and responsibilities consistent with providing a psychosocial environment to assist with the rehabilitation and community integration of clients. The AMHW will support clients who experience persistent and chronic homelessness with a focus on supporting them to achieve housing stability.

Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed in the last six months. Interested applicants who reside in the City of London and are in need of a PVSC are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 5 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

WHAT WE'RE OFFERING:

  • What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ( $30.01 per hour for Level 1 – Direct Service, Regulated):
    • Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
    • Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)
    • For internal applicants, grid level placement will occur as per the Collective Agreement
  • When you'll work - 70 hours over two-week period; Scheduled within the hours of 8:30 am to 4:30 pm, Monday-Friday, with one unpaid hour for lunch
  • What your benefits will be - 9% in lieu of benefits
  • Your pension plan - Immediately eligible for optional enrolment in Healthcare of Ontario Pension Plan (HOOPP)

HOW YOU WILL BE MAKING A DIFFERENCE:
Designated Primary Worker for clients housed in Exeter region:

  • Ensure Electronic record and paper file is complete and up-to-date
  • Ensure client’s needs are being met and that each is supported to maintain housing
  • Provide updates at Team Meetings

In addition to the responsibilities of the Primary Worker, AMHW’s will fulfill the following with all clients;

Housing Stability:

  • Provide support based on presenting need of the client
  • Use assessments to base goals based on their strengths, abilities, needs and preferences
  • Motivate and support clients to work towards achievement of their needs and/or goals
  • Assist and coach clients in the learning and management/achievement of life skills including activities of daily living such as personal hygiene, chores, budgeting, socialization, etc. alongside them in their apartment
  • Advocate, refer and connect clients with appropriate services, supports, and resources (formal and informal) based on their needs
  • Support clients with physical and mental health concerns, and liaise with clinical staff
  • Coordinate recreational community outings and activities to enhance clients’ quality of life and further connect clients to their community
  • Champion each client’s individual capacity for independent community living
  • Provide health teaching about mental illness and symptom management
  • Assist clients to successfully remain in the apartment, providing prevention and diversion strategies to avoid homelessness
  • Crisis prevention and intervention; reducing the utilization of EMS, ER visits, days in hospital, days incarcerated
  • Manage interpersonal and day to day interactions in each house

Assessment and Goal Planning:

  • Assess clients’ strengths, abilities, needs and preferences as well as social and environmental factors impacting the client, and any vulnerability and risk factors
  • Ensure ongoing assessment are completed and reviewed according to policy
  • Include and encourage clients’ natural supports (e.g. Family, significant friends) in goal planning, clarifying the roles each plays towards each client’s rehabilitation
  • Regularly solicit feedback from the client and relevant community supports regarding the client’s mental health and progress towards his/her goals
Communication and Decision Making:
  • Maintain open communication with client’s professional and natural supports in regards to client’s rehabilitation
  • Make decisions both independently and collaboratively
  • Complete required documentation related to clients as per CMHA - TVAMHS standards
  • Complete other required documentation (e.g. monthly time sheets, vacations requests, education requests, travel claims, expense claims) within the required time frames
  • Provide input into decisions with team concerning operations of the program, policies of CMHA - TVAMHS and accreditation standards
  • Professionally liaise with community workers and agencies regarding the housing program
  • Ensure effective communication with Director, Managers, staff team, and external treatment teams concerning each client’s progress
  • Identify, promote resolution, and management of all emergency situations concerning clients, staff, and property
  • Knowledge of issues pertaining to confidentiality, such as the shared Common Client Record (CCR) and consent to release personal information process to ensure client’s confidentiality is maintained at all times

WHAT YOU'LL NEED TO APPLY:

We require the following qualifications:

  • Degree or Diploma in the social science field (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
  • Valid Driver's Licence and access to a vehicle with at least $2 million in liability insurance to transport clients
  • Ability to attend work regularly

We prefer the following qualifications:

  • Current First Aid and CPR certification
  • ASIST certificate
  • Previous Peer Support experience, including paid, volunteer, and/or placement positions
  • Minimum of one year of work experience in the mental health and addictions field
  • Experience working with homeless/marginalized populations
  • Knowledge of Psychosocial Rehabilitation
  • Knowledge of psychiatric medications
  • Knowledge of community resources
  • Knowledge of electronic documentation methods
  • Health and safety knowledge
  • Crisis intervention and prevention
