Location: St. Mary's Medical Center- Blue Springs, MO

The Acute Rehab Admissions Liaison serves as a vital link between patients, healthcare providers, and rehabilitation services, playing a key role in the continuum of care. This position ensures seamless coordination and communication from the onset of a patient’s illness or injury through recovery and transition to rehabilitation.

In this role, the Liaison follows patients throughout their hospital stay, assessing for acute rehabilitation eligibility and advocating for appropriate level-of-care placement. Responsibilities include coordinating admissions, facilitating smooth care transitions, and educating patients and families on the rehabilitation process, expected outcomes, and available services.

Responsibilities

Essential Duties and Responsibilities (includes, but is not limited to):

  • Evaluate patients for acute inpatient rehabilitation eligibility using on-site and chart review, applying medical necessity and functional criteria
  • Build relationships with case managers, discharge planners, physicians, and nursing staff to drive patient referrals and support care transitions
  • Educate patients, families, and clinical teams on rehabilitation services, therapy outcomes, and post-acute care options while advocating for an appropriate level of care
  • Coordinate admissions, patient placement, insurance verification, and prior authorizations to ensure smooth transitions to the acute rehab unit (ARU)
  • Monitor referral pipeline, census, conversion rates, and admission metrics to identify barriers and grow referral networks

Qualifications

Before we go any further, we do have some deal-breakers You must have

  • Graduated from an accredited School of Nursing, Physical Therapy, Occupational Therapy, and Speech Pathology.
  • Bachelor's degree in a health-related field or obtained within the specified time of hire or equivalent experience.
  • 3 years of experience in the rehab area.
  • BLS Course forms the American Heart Association (AHA) certification upon hire and maintains current certification
