Job Description

Deaconess Illinois Cardiology is seeking a highly motivated and skilled Acute Care or Family Nurse Practitioner or Physician Assistant to join our dynamic and growing Cardiology Outpatient Clinic. This is an exciting opportunity to work alongside two experienced Interventional Cardiologists and a team of APPs in a fast-paced, collaborative environment. The full-time position is primarily outpatient-focused, offering exceptional patient care for individuals with cardiovascular conditions. Candidates should be licensed in the State of Illinois and certified by the ANCC/AANP as a Nurse Practitioner or certified by the NCCPA as a Physician Assistant.

Key Responsibilities:

• Provide high-quality, patient-centered care to cardiology patients in an outpatient setting.
• Conduct comprehensive health assessments, including history-taking, physical examinations, and diagnostic testing.
• Order and interpret lab tests, EKGs, and other diagnostic procedures.
• Develop, implement, and manage treatment plans in collaboration with supervising cardiologists and the healthcare team.
• Assist with patient education, providing counseling and guidance on heart disease prevention, lifestyle modifications, and medication management.
• Follow up with patients’ post-procedures, monitor chronic conditions, and adjust treatment plans as necessary.
• Coordinate care with other specialists and healthcare providers involved in the patient’s treatment.
• Participate in quality improvement initiatives, ensuring adherence to best practices and optimal patient outcomes.
• Document patient interactions, assessments, and treatment plans in electronic health records (EHR) accurately and timely.
• Provide clinical support to the cardiology team, including assisting with urgent patient needs and coordinating referrals.

Bonus:

Potential for a performance-based bonus, subject to eligibility criteria and achievement of defined metrics. Details regarding bonus eligibility and performance goals will be provided during the interview process or upon hire.

Compensation:

Hourly Range $59.00-$62.50 (median hourly rate $60.82)
