Job Description

Welcome to Montage Health’s application process!

The Activity Coordinator has the responsibility for developing and carrying out the Activity function at the Hospice House. The Activities Department follows and meets the Title 22 Regulations. Job responsibilities include bit not limited to timely assessments and documentation of socialization and activity needs of the patient; maintaining and upgrading inventory and supplies to carry out the activity function; educating and training the Hospice House staff to the role and function of the Activities Department; provide and develop activities for a Short Stay Activity Program. These activities include but are not limited to the following: social activities, indoor and outdoor activities, religious visits, residential council, creative activities, educational and exercise activities.

Experience/Education

One of the following:

-Have two years’ experience in a social or recreational program within the past 5 years, one of which was full-time in a patient activities program in a healthcare setting.

-Graduate of an accredited school of Occupational Therapy, Recreational Therapy or Occupational Therapy Assistant.

-Have satisfactory completed at least 36 hours of training in a course designed specifically for the position.

Required education- As appropriate for education and experience above.

Licensure/Certifications

American Heart Association Healthcare Provider BLS certification is required.

40 hours per week; 8am-4:30pm; Schedule may change based on events and activities occurring.

Pay Range (based on years of applicable experience):

$31.01

to

$41.50

The hours employees work determine when a shift differential is paid.

Hourly Evening Shift Differential: $2.01Hourly Night Shift Differential: $2.95
