Job Description

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Activities Coordinator – Joyful Living Adult Day Health
Bilingual proficiency in Mandarin or Cantonese is required for this role.

What You'll Do
• Plan and facilitate engaging group and individual recreational activities based on each participant's interests, abilities, and goals
• Develop and implement person-centered activity programs that promote socialization, independence, and quality of life
• Collaborate with the interdisciplinary team to support individualized care plans and participant engagement
• Provide guidance and support to staff on activity implementation and engagement strategies
• Maintain curriculum binders with creative, age-appropriate, and therapeutic activities
• Ensure a safe, welcoming, and inclusive environment for all participants
• Monitor participant involvement and document activity participation as required

Qualifications
• High school diploma or equivalent required
• Valid driver's license with a clean driving record required
• Minimum of two (2) years of experience working with individuals with developmental disabilities
• Bilingual in Mandarin or Cantonese is required
• Strong organizational, communication, and interpersonal skills
