Job Description

$22.56 - $31.54 Pay range

The Club Unity Program Coordinator position at Lake Forest Placeis a unique opportunity to plan and facilitate programs for residents with early stage memory loss who reside in Independent Living. Modeled after an adult day services approach to well-being, this program provides a structured day of engagement for our Independent Living residents who need support managing their days.

Primarily Monday - Friday Schedule.

Essential Functions:

  • Plan and facilitate a full day of engaging programs tailored to the cognitive and physical abilities of participants in the Dementia Enrichment Program, encouraging their highest functional abilities.
  • Support and supervise a Program Specialist who facilitates programs, ensuring high-quality, resident-centered activities.
  • Evaluate resident participation and outcomes, communicating regularly with families to provide updates and gather input.
  • Assist with recruiting new participants for the program, as well as supporting their care and transitions by collaborating with other disciplines within Lake Forest Place.
  • Collaborate with the Programs Department to coordinate a variety of events and ensure alignment with organizational goals.
  • Manage program operations, including creating innovative and engaging activities aligned with the Presbyterian Homes Choices Program's 7 Dimensions of Wellness.
  • Maintain documentation and track attendance according to established program policies.
  • Develop professional relationships with participants, learning their stories, and responding to their preferences and needs.
  • Recruit, train, and assign volunteers to support program goals and participant engagement.
  • Utilize software tools such as Caremerge, Touchtown, MyUnity, and Microsoft Office to enhance communication and performance.
  • Stay current with required dementia education and ensure program staff complete their training in a timely manner.
  • Adhere to Presbyterian Homes Corporate Compliance, HIPAA, and confidentiality policies, demonstrating concern for residents' well-being.

Skills/Qualifications:

  • Leadership skills with the ability to work independently and collaboratively.
  • Resourceful and creative in planning activity programs.
  • Basic computer skills with a willingness to learn audio-visual technology.
  • Strong written and verbal communication skills in English.
  • Energetic, positive, and adaptable to changing conditions.
  • Availability to work evenings, weekends, and holidays as required.

Education and Training:

  • Bachelor’s Degree or equivalent experience in fields related to Human Services preferred.
  • Experience or education in therapeutic music, arts, or recreation for dementia populations is preferred.

Physical Demands:

  • Standing and walking for extended periods.
  • Pushing supply carts and assisting in fitness classes.
