This position provides assertive community treatment services to clients assigned to the Assertive Community Treatment (ACT) program who have a severely persistent mental illness. These services are provided to assist in resolving or minimizing the effects of a client's impairment through a wide range of skills, including assess­ment, monitoring, crisis intervention, education, linking and training in community resources, assistance in daily living skills, and obtaining necessary services for meeting basic human needs and entitlements.

Key Responsibilities & Role Highlights

  • Provides assertive community treatment services to clients assigned to the ACT Program in accordance with the State mandates and established Agency and program policies, procedures, philosophy and client individual service plan.
  • Maintains client confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures.

Education & Experience Requirements:

  • Bachelor's degree OR a QMHS 3+ with high school diploma required.
  • At least one year experience working with SPMI clients in a community setting.
  • Prior ACT experience preferred. Must obtain CPR and CPI certification following hire.
  • LSW or LPC preferred.
  • If not licensed, must meet Ohio MHAS criteria for qualified mental health specialist (QMHS).
  • Must demonstrate knowledge of mental illness and the ability to establish and maintain therapeutic relationships with clients.
