The Licensed Practical Nurse will serve clients by conducting various nursing duties across numerous outpatient psychiatry, addictions, & housing services while implementing & maintaining the therapeutic client-clinic staff relationship.
Essential Job Functions:
Functions as a resource person for nursing & medical issues. Oversees infectious disease control, client hygiene, and related client & facility health issues, under the supervision of nursing leadership.
Performs all appropriate nursing assessments per service line requirements, utilizing a high level of clinical competence. Examples include but are not limited to: AIMS, physical & mental health assessments, and psychiatric screeners.
Administers or oversees administration of medications via various methods (oral, injection, & others as indicated) according to service line requirements. Carries out complete documentation of medication administration per service line requirements. Conducts Medication Administration Record (MAR) reviews as indicated and daily medication counts according to regulatory & service line standards. Monitors house stock of medications & refill status for housing clients. Prepares pass medications as needed according to service line. Conducts medication destruction as needed and on a routine basis, according to service line.
Communicates with clients regarding medication questions & needs; stages refill requests to providers according to standard operating policies & procedures.
Rooming of clients, verification of client data / medical history, confirming purpose of visit, obtaining necessary physical preliminary requirements such as blood draw, urine sample, height, weight, blood pressure, pulse, and temperature.
Completes all required documentation in a timely manner as indicated per our company policies & procedures. Examples of required documentation may include but are not limited to: nursing assessments, all physical & spoken interventions with clients (such as phone calls, check-ins, and laboratory tasks), conducts nursing-led educational groups & one-on one sessions, and completes & reviews client safety plans & psychiatry screeners as indicated.
Guarantees that all medical & medication records are up-to-date and accurate. Ensures appropriate prior authorization paperwork is completed for maintenance of client medication compliance.
Assists as needed with general lab & phlebotomy duties such as venipuncture & obtaining urine samples while following universal infection control precautions. Enters lab requisitions electronically via outside lab processing company. Coordinates communication of results of all labs performed to ordering providers via entry into the EHR. Communicates critical results immediately via entry into the EHR, email/instant messaging, & verbally, as appropriate to providers.
Functions as a resource person for nursing & medical issues. Assists, according to service line with ensuring medical appointments are made & kept. Is available as on-call resource for needs after-hours per service line requirements, as assigned.
Interviews clients, families &/or guardians to gather pertinent nursing & medical history. Secures client records from outside agencies when necessary.
Participates in all required meetings as assigned.
Performs other related duties as required, assigned, &/or delegated by nursing leadership.
Knowledge, Skills, and Abilities:
Secures client information and maintains confidentiality by completing and safeguarding medical records. Knowledge and implementation of safeguarding protected health information by methods such as maintaining private user names and passwords, encryption, and other methods as indicated.
Ability to read & interpret documents such as lab results, notes & documents within an electronic health record, safety rules, operating & maintenance instructions, and procedure manuals.
Ability to write routine reports & correspondence clearly & effectively via multiple methods, in various platforms. Communicates effectively & therapeutically to clients, family members, employees, & other contacts within the community in-person, via telephone, and via video conferencing.
Ability to add, subtract, multiply, & divide all units of measure using whole numbers, common fractions, & decimals.
Ability to apply common-sense understanding to carry out instructions provided in written, oral, or diagram form. Ability to problem solve involving several concrete variables in standardized situations.
Shall be able to follow direction and accept supervision.
Experience and Education Qualifications:
Valid licensure as a practical nurse; one year of experience preferred.
Supervisory Requirements:

Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
Completion of New Hire Orientation at the beginning of employment.
All training requirements including Relias at the beginning of employment and annually thereafter.
Current driver’s license, acceptable driving record and current auto insurance.
Physical Requirements:
