Reporting to the Director of Business Development, this role is responsible for expanding membership through strategic community outreach, referral source development, and relationship management. The ideal candidate will combine industry knowledge with strong networking and marketing abilities to drive sustainable growth and strengthen community partnerships.

Responsibilities:

• Develop and maintain a targeted list of referral sources, including physician offices, home care agencies, assisted living facilities, and community organizations, aligned with the company’s referral strategy.

• Conduct daily outreach visits to strengthen relationships with new and existing referral partners, educate them on the Plan’s unique value and benefits, and drive consistent member referrals.

• Promote the Plan within the community through pre-planned marketing activities, presentations, and ongoing engagement efforts to increase awareness and membership growth.

• Maintain accurate documentation of outreach activity and referral leads, ensure proper follow-up, meet assigned quotas and goals, and actively participate in team and company meetings.

Skills:

• Strong understanding of Medicaid, Medicare, MLTC, MAP, and DSNP guidelines.

• Excellent communication, presentation, and relationship-building abilities.

• Highly organized with strong attention to detail and documentation skills.

• Self-motivated, results-oriented mindset with a strong drive to achieve growth targets.

Requirements:

• Prior experience in Home Health Care preferred.

• Knowledge of Medicaid/Medicare regulations and managed care plans required.

• Proven ability to build and maintain referral source relationships.

• Ability to work independently while collaborating with leadership.
