DEPARTMENT: Access and Marketing

POSITION SUMMARY:
Responsible for intake and referral processing.
WORKING RELATIONSHIPS:
REPORTS TO: Access Supervisor/Team Leader, Admissions
SUPERVISES: None
INTERPERSONAL RELATIONSHIPS: All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.
DUTIES AND RESPONSIBILITIES:
- Performs intake and processes referrals.
- - Screens referrals for eligibility for hospice services.
- - Arranges DME, supplies, etc. Liaisons with MDs, referral sources, etc. If LVN, arranges medications.
- - Inputs data, POC, demographics, etc. into Suncoast.
- - Schedules admission visits.
- - Provides staff/IDT with patient admission information and documentation.
- Performs data entry (excel, EMR) and maintains records and information for patients being admitted to Hospice Austin.
- - Processes medical records requests
- - Receive incoming faxes (e.g. lab orders, med refills, therapy notes, etc.) and notifies/forwards to appropriate parties (e.g. Team Leader, MD, HIM, RN, etc.).
- - Scan patient records and attach in Suncoast Library.
- - Assists in auditing/processing admissions charts.
- Other duties as assigned.

QualificationsMINIMUM TRAINING, EDUCATION AND EXPERIENCE REQUIREMENTS:
- High School diploma or equivalent.
PREFERRED QUALIFICATIONS:
- One year clerical experience in a medical environment required.
- Experience in hospice/home health admissions preferred.
