Description

The University of Iowa Health Care department of Family Medicine is seeking an Access, Continuity, & Empanelment (ACE) Coordinator. The ACE Coordinator provides operational and administrative support for provider panel management, patient attribution, continuity-of-care activities, access management initiatives, EHR data integrity, reporting, and stakeholder communication across University of Iowa Health Care. The position supports the department, providers, clinic leaders, patient access teams, and operational partners through panel management, analytics, peer chart review coordination, and access optimization efforts. Additional Duties: Provider Panel and Empanelment Support· Coordinate and process new patient primary care provider (PCP) assignments.· Facilitate PCP reassignment activities according to established departmental procedures.· Monitor provider panel status and capacity and assist leadership with recommendations regarding panel availability.· Coordinate patient reassignment activities associated with provider departures, clinic relocations, and provider transitions.· Support management of patient attribution changes related to provider leaves of absence.· Coordinate temporary provider coverage plans and patient redistribution activities.· Maintain panel assignment records and supporting documentation. Perform weighted panel calculations and maintain associated reports.· Support implementation and maintenance of standardized patient attribution and empanelment practices.· Assist with panel management initiatives designed to improve patient access and continuity of care. Data Integrity and Information Management· Conduct routine audits of provider assignment data within the EHR.· Perform panel reconciliation activities to ensure accurate patient-provider attribution.· Monitor inactive patient populations and facilitate appropriate panel maintenance actions.· Identify and resolve attribution discrepancies and data quality concerns.· Process updates related to deceased patients and patients who have transferred care.· Reconcile payer attribution data with EHR attribution records.· Maintain provider full-time equivalent (FTE), panel management, and attribution databases.· Collaborate with operational, clinical, and informatics teams to improve data accuracy and consistency.· Generate and maintain reports supporting panel management and access initiatives.· Coordinate peer chart review activities to support quality, continuity-of-care, and documentation improvement initiatives.· Maintain peer chart review tracking, reporting, and documentation records.· Prepare chart review assignments and support follow-up activities to ensure timely completion of reviews.· Compile and distribute peer chart review reports and identify documentation, attribution, or workflow trends for operational and quality improvement discussions. Reporting and Analytics Support· Collaborate with data analytics teams to create and maintain panel management dashboards and reports.· Monitor and report provider panel size and weighted panel metrics.· Track patient access measures including appointment availability and new patient wait times.· Support continuity-of-care reporting and analysis, including coordination of template standardization.· Monitor same-day access capacity and utilization trends.· Prepare summaries, reports, and presentations for operational leaders and stakeholders.· Assist in identifying trends and opportunities related to access, continuity, and provider capacity. Communication and Coordination· Serve as a resource regarding provider panel status, patient attribution, and empanelment processes.· Communicate panel updates and access information to clinic managers, providers, clinic teams, Patient Access Center staff, and referral teams.· Coordinate implementation of panel-related operational changes and workflow updates.· Assist in developing educational materials, process documentation, and communication tools.· Facilitate collaboration among operational, clinical, scheduling, and access teams regarding panel management activities. External Information Management· Coordinate updates to provider information displayed on organizational websites.· Assist in maintaining accurate provider directory information.· Support payer directory and provider network updates within organization.· Provide provider panel status and access information to marketing and communications partners.· Other duties as assigned. Qualifications

Required Qualifications:· Bachelor's degree in Healthcare Administration, Public Health, Business Administration, Health Informatics, Information Systems, or equivalent combination of education and experience. · Experience working in healthcare operations, ambulatory clinic operations, patient access, scheduling, health information management, analytics, population health, or related healthcare support functions.· Experience working with electronic health record systems and healthcare databases.· Experience gathering, maintaining, analyzing, and reporting operational data.· Demonstrated experience coordinating multiple priorities · Excellent written, verbal, and interpersonal communication skills.· Strong organizational skills and attention to detail. Desirable Qualifications:· Experience with patient empanelment, patient attribution, or provider panel management.· Knowledge of ambulatory care and primary care operations.· Experience using Epic and related reporting tools.· Experience creating operational dashboards and reports.· Knowledge of access management, continuity-of-care principles, and provider capacity planning.· Ability to analyze complex data and communicate effectively.· Demonstrated commitment to process improvement and operational excellence.· Experience collaborating with multidisciplinary healthcare teams.
