Job Description

Job title: Aboriginal Health Worker

We are seeking a dedicated Aboriginal Health Worker to provide culturally safe Case Management, Transition of Care, and Friends and Family support to Aboriginal parents and/or carers participating in a non‑residential day rehabilitation program. This role prioritises families with children aged 0–5 years and extends support to families with children up to 16 years of age, for those involved in AOD services and child protection interventions.

The successful applicant will work within our stepped‑care, transdisciplinary model to promote the health, wellbeing, and long‑term outcomes of clients and their families. As part of the Yadhabadiwa program, Aboriginal Health Workers will work flexibly across Transition of Care, Friends and Family support, and Case Management, contributing to coordinated, holistic care.

Skills:

- Strong understanding of Aboriginal culture, values, and community

- Excellent communication and interpersonal skills

- Ability to provide culturally responsive care

- Demonstrated knowledge of AOD (Alcohol and Other Drugs) interventions

- Experience in child protection interventions

Responsibilities:

- Provide case management for clients participating in the non-residential day rehabilitation program

- Conduct assessments to determine client needs and develop individualised case plans

- Coordinate services with other health, social emotional wellbeing services

- Monitor progress towards goals outlined in case plans

- Provide education on harm reduction strategies, parenting skills, etc.

- Co facilitate recovery and therapeutic interventions

Qualifications:

- Certificate IV in Community Services, AOD/Mental Health or related fields

- Previous experience working as an Aboriginal Health Worker or similar role

- Knowledge of AOD interventions and child protection protocols

- Current First Aid/CPR certification

- Valid driver's license

- Current and Valid Working with Children's Check
