Job Description

The Aboriginal Health Practitioner (AHP) – Chronic Conditions is a vital member of Wunan Health’s’ multidisciplinary team (MDT), providing culturally safe, person-centred care for Aboriginal individuals and families in the prevention and management of chronic and complex health conditions.

Working under the clinical supervision of the Chronic Conditions Nurse (CCN); the AHP delivers clinical care and contributes to the coordination, case-management, service navigation and education to equip people with the skills, confidence, and supports needed for day-to-day living with a chronic condition.

This position works closely with the Workforce Cultural Safety Coordinator and AHA, to identifying and address barriers to culturally safe access to allied health services in Kununurra; and provides cultural guidance to ensures all aspects of the program are responsive to Aboriginal ways of knowing, being, and doing.
