Job Title: 2S&LGBTQ+ Program Facilitator
Location: Niagara Falls Community Health Centre
Work Schedule: Part-time, up to a maximum of 18 hours/week
Salary: $27.9745/hr - $30.5703/hr

Position Summary:
The 2S&LGBTQ+ Community Programs Coordinator will develop, coordinate, and deliver inclusive, community-informed programs and initiatives that promote health, belonging, connection, and well-being for 2S&LGBTQ+ communities across Niagara. Working as part of the Population Health team, this role will engage with community members and partners, facilitate programming and outreach, and contribute to organizational learning and health equity initiatives.

Key Responsibilities:
- Develop, coordinate, implement, and evaluate 2S&LGBTQ+ support groups, educational workshops, community initiatives, health promotion programs, and special events.
- Facilitate inclusive and engaging programming that promotes health, belonging, resilience, peer connection, and community engagement.
- Coordinate community events, awareness campaigns, educational initiatives, and opportunities for peer support and community connection.
- Facilitate focus groups, community consultations, listening sessions, and engagement activities to identify emerging needs, barriers, priorities, and opportunities for service improvement.
- Utilize participant feedback and community-informed perspectives to support program planning, evaluation, and continuous quality improvement.
- Apply a population health lens to program development by identifying trends, strengths, and opportunities to improve services and supports for 2S&LGBTQ+ communities.
- Develop and maintain partnerships with community organizations, schools, service providers, and other stakeholders.
- Participate in outreach activities and represent NFCHC at community events, planning tables, and collaborative initiatives.
- Support organizational learning, allyship, and capacity-building related to 2S&LGBTQ+ inclusion and affirming practices.
- Collaborate with interdisciplinary team members to support Population Health priorities and strategic objectives.
What We’re Looking For:
- Completion of a diploma or degree in Social Work, Child and Youth Care, Community Development, Social Service Work, Health Promotion, Public Health, Education, Psychology, Sociology, or another related discipline, or an equivalent combination of education and experience.
- Experience facilitating groups, workshops, educational sessions, or community-based programming.
- Experience working with 2S&LGBTQ+ individuals and communities.
- Demonstrated understanding of health equity, anti-oppressive practice, and the social determinants of health.
- Strong group facilitation, presentation, and community engagement skills.
- Experience developing community partnerships and collaborating with diverse stakeholders.
- Excellent interpersonal, organizational, and communication skills.
- Ability to work independently and collaboratively within an interdisciplinary team environment.
- Commitment to inclusion, allyship, equity, and continuous learning.
- Knowledge of community development and population health approaches considered an asset.
- Lived experience as a member of the 2S&LGBTQ+ community is considered a strong asset.
Additional Requirements:
- Ability to work flexible hours, including evenings and weekends.
- Valid Ontario Driver's Licence and access to reliable transportation considered an asset.
- Vulnerable Sector Check required.
