Responsibilities: Under the supervision of the Case Management Services Coordinator of Adult Mental Health Services Team 3, serves as a professional performing a wide range of direct and indirect mental health services to consumers with a chronic mental illness or co-occurring disorders. Provides assessments; treatment planning; case management services; crisis intervention; mental health services; referral and linkage to various other CMHA-CEI programs and/or services in the community; minimal outreach services to consumers with multiple needs; and facilitation of group activities. Responsible for detailed case documentation. Services may occur both in office and the community. Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures.
Requirements: Bachelor's degree in Social Work, Psychology or other related field required. A minimum of one year of professional experience with adults with primary diagnoses of a serious and persistent mental illness and/or co-occurring disorder is required. On a case-by-case basis, may assess and consider an applicant with two years of paraprofessional work experience with adults with primary diagnoses of a serious and persistent mental illness and/or co-occurring disorder. Must be able to operate a computer and have the ability to gain proficiency in CMHA-CEI’s computer systems. Ability to communicate effectively both in writing and verbally is required. Possession of a valid Michigan driver’s license and access to reliable transportation for job related use is required.
Conditional Employment Requirements: Employment offers for this position will be contingent upon the results of a background verification and credentialing procedure consisting of: Michigan State Police clearance check, verification of a professional license, Medicaid/Medicare verification, verification of an acceptable Michigan driving record, educational background check and verification of an acceptable work history. Must be able to pass pre-employment drug screen.

Salary/Hours: $27.12 – $30.37 per hour. 40 hours per week.
