The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Primary Health Nurse II provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.

Qualifications

Education
Graduate of approved Registered Nursing education program, BN preferred.
Must be a member in good standing with CRNM

Skills, Expertise and Experience
A minimum of five (5) years’ experience working as a Registered Nurse
A minimum of three (3) years’ experience working in a community health setting
Knowledge and Experience working with Students and/or Volunteers
Current CPR Certification at Basic Rescuer Level

Proven knowledge and ability in critical thinking, decision-making, problem-solving, and crisis management
