Provide an ongoing program of activities designed to meet the interests and the physical, mental and psychological well-being of each patient. Discharges duties in a professional manner while demonstrating values of respect, dignity, integrity and compassion. Actions are consistent with hospital policies and procedures and current standards/regulations governing nursing services and long term care.

MINIMUM QUALIFICATIONS

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. High School diploma or equivalent

2. Obtain certification in Basic Life Support within 30 days of hire date.

To be completed as established by department: (Administered at Department level)

1. Completion of recreation specialist training course approved by State of West Virginia within 6 months.

PREFERRED QUALIFICATIONS

EXPERIENCE:

1. Clerical experience

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Performs data entry and discharge tracking.

2. Provides care and education appropriate to the age of the patients served. (Adult 18-79 years, Geriatric 80+ years)

3. Maintains an understanding of treatment necessary for each patient and provides appropriate care based on their age specific needs.

4. Maintains communication in a professional manner including, but not limited to:

  • Establishes and maintains good rapport with patients, families, volunteers, and peers.

  • Provides emotional support to patient and family.

5. Attends in-services and other educational programs as required with special emphasis on programs related to the geriatric patient.

6. Attends and participates in at least three staff meetings.

7. Determines areas of self-improvement; writes developmental goals on performance appraisal yearly.

8. Provides activity in-service program for department.

9. Reviews and initials meeting minutes and postings in department.

10. Supports hospital and departmental cost containment efforts.

11. Plans, implements and evaluates activity program.

12. Provides individual and group activities which promote the maintenance to enhancement of the patient's quality of life.

13. Accurately completes specific areas of the minimum data set (MDS).

14. Develops individualized plan of care with the patient.

15. Establishes measurable goals, evaluates plan of care and adjusts plan as needed.

16. Prepares calendar of activities one month in advance :

  • Activities vary per month with special events each month

  • Activities are posted monthly

17. Prepares and maintains activity records and reports.

18. Transports patients to and from activities.

19. Coordinates and supervises activities of volunteers.

  • Assists in the orientation and development of volunteers for Skilled Care.

  • Coordinates the daily activities and duties of the volunteers.

20. Orders supplies and equipment for activity program while working within departmental budget.

21. Participates in continuous improvement activities.

22. Fosters relationships with community groups and organizations to provide activities for the skilled patient.

1. This position requires a wide range of body motion. Moderate to heavy physical activity may be required. This includes lifting, pushing a loaded cart, and exercise type activities.

2. Ability to walk and stand for extended periods required.

3. Finger dexterity and hand/eye coordination necessary.

4. Occasionally transports and carries items of moderate weight.
