The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

MINIMUM REQUIREMENTS
Education:
Associate's degree or graduate from an accredited school of nursing.Bachelor's of Science in Nursing (BSN) preferred.
Experience:
Threeyears in an acute care setting.Previoustelemetry experiencerequired

CPR Requirements:

ESSENTIAL FUNCTIONS
Delivers quality care to patients through a team effort in working with others through the nursing process of assessment, diagnosis, planning, implementation, evaluation, education & care coordination
Delivers nursing care in-home to patients enrolled in the Hospital at Home program.
Within the on-site Command Center, conducts virtual nursing assessments via video conferencing technology for patients enrolled in the Hospital at Home program
Monitors patient status remotely using connected devices and electronic health record (EHR) system data.
Documents all virtual interactions in the EMRin accordance withNJ Board of Nursing and CMS standards.
Triages patient care needs and escalates appropriately.
Assurespatient safety by executing appropriate policies& procedures
Providesappropriate supportfor pain management, pharmacological and non-pharmacological measures.Assistpatient/family inidentifyingindividualized comfort measures
Collaborateswith in-home clinical staff, providers, and other members of the healthcare team to ensure real-time updates and care delivery.
Mobilizes resources in complex cases to maximizepatientscontrol and participation over his/her own recovery.
Provides information and interpretation of the patient's condition and offers coping mechanisms. Provides these to bothpatientand family.
