Job Description

The Housing Support Worker is a community-focused role at Central Commons dedicated to enriching tenants' daily lives through robust recovery programming and activity facilitation. Guided by Psychosocial Rehabilitation (PSR) principles, this on-site position assists in carrying out individual support plans, outreach, and skill-building to help Tenants maintain their formal and informal support networks.

Working under the Housing and Supports Manager, this role provides proactive intervention and collaborative support to the Tenant Relations Coordinator (TRC) team. The ultimate goal is to drive Tenant engagement and provide the necessary support to ensure all tenants successfully sustain their housing and achieve their recovery goals.

Qualifications:
o Post-secondary education related to healthcare or human services preferred
o Current First Aid Certification required

Key Responsibilities

  • Daily Living Support: Model and teach Activities of Daily Living (ADLs) to empower tenants toward independence and assist them in maintaining their personal living spaces.
  • Direct Outreach: Conduct regular, proactive check-ins to identify immediate support needs and prevent Tenant isolation.
  • Household Facilitation: Assist Tenants in organizing and completing routine household tasks to ensure a safe, clean, and collaborative communal environment.
  • Housing Stability: Proactively identify and address daily barriers to housing; provide hands-on support to ensure Tenant s successfully sustain their tenancy.
  • TRC Collaboration: Provide essential on-site support to the Tenant Relations Coordinator (TRC) team regarding rent collection, move-in/move-out coordination, and recovery-oriented enforcement of building rules.
  • Risk Observation: Monitor Tenants well-being and provide timely, objective reports on any behavioral changes or safety concerns to the clinical and management teams.
  • Tiered Intervention: Act as the primary point of contact for lower-tier tenant issues, effectively balancing immediate crisis response with the Tenant 's long-term recovery goals.

  • Team Player: Ability to work collaboratively within the housing team to achieve shared goals.
  • Flexibility: Ability to be flexible to meet different shift schedules as required.
  • Balanced Focus: Ability to respond to immediate resident issues while maintaining work on long-term goals.
  • Clinical Observation: Ability to provide a functional assessment of observed behavior.
  • Teaching: Thorough knowledge of life skills and the ability to teach the same to residents.
  • Communication: Exceptional skills for managing reporting and inter-site coordination.
  • Mission Driven: Ability to draw on Eden’s vision, mission, and values to foster a recovery-focused housing culture.
