Job Title: Licensed Vocational Nurse (LVN)
Hourly Rate: $33.00/hr.
Job Status: Part-Time
Hours: 26 hours per week
Work Location: Antelope, CA

Here's What You'll Do:
The Licensed Vocational Nurse (LVN) supports the health, safety, and well-being of individuals with intellectual and developmental disabilities. The LVN provides nursing care, administers medications and treatments, monitors individuals for changes in their physical or behavioral condition, and advocates for their health and quality of life in accordance with their Individual Program Plan.
The LVN works collaboratively with administrators, physicians, direct support professionals, behavioral health professionals, and other members of the care team to ensure individuals receive appropriate and person-centered care.
Essential Functions:
• Administer medications to individuals and follow the five rights of medication administration.
• Audit medications during each new medication cycle and verify medication orders and the five rights.
• Attend residents' and participants' medical appointments and communicate concerns, changes, and relevant health information to the attending physician and care team.
• Monitor and document residents' and participants' health status and report significant changes or concerns.
• Assess wounds and assist with dressing changes and breathing treatments, as needed.
• Perform G-tube care and feeding procedures, as necessary and within the LVN's scope of practice.
• Perform intramuscular injections when necessary and as permitted by applicable regulations and the individual's care plan.
• Monitor individuals' level of consciousness, neurological status, responsiveness, and orientation following seizures, injuries, or other traumatic episodes.
• Assess and identify signs and symptoms of infection and report concerns promptly.
• Perform basic physical assessments, including monitoring vital signs and recognizing normal and abnormal findings.
• Observe and report unusual behaviors, injuries, changes in condition, and other health concerns to the Administrator and appropriate members of the care team.
• Complete and maintain accurate daily documentation and client notes.
• Assist individuals with activities of daily living, including bathing, grooming, meal preparation, cleaning, and other personal care needs as appropriate.
• Provide support and redirection during behavioral episodes while maintaining the individual's safety, dignity, and well-being.
• Participate in community outings and activities with individuals as needed.
• Maintain confidentiality and comply with applicable laws, regulations, policies, and reporting requirements.
• Follow emergency, medication, infection control, and abuse reporting protocols.
• Perform other related duties and assignments as required.
Education and Experience:
• High school diploma or GED.
• Must possess a valid California Licensed Vocational Nurse (LVN) license.
• DSP I and DSP II certificates.
• Current CPR/First Aid certification.
• CPI (Crisis Prevention Intervention) certification preferred or willingness to obtain.
• At least six months of experience working as an LVN in a residential care facility or similar setting preferred.
• At least one year of experience supporting adults with intellectual and developmental disabilities preferred.
• Experience with medication administration, G-tube care, injections, wound care, and other nursing procedures preferred.
Physical Requirements:
• The LVN position will typically work in a residential, office, and community-based environment, with occasional work outdoors and in small or enclosed spaces.
• Frequent sitting, standing, walking, grasping, feeling, talking, hearing, and repetitive motions.
• Occasional lifting of up to 50 lbs.
• Ability to bend, twist, stoop, squat, push, pull, reach, kneel, crouch, and crawl as needed.
• Ability to assist individuals with activities of daily living and personal care.
• Ability to travel between work locations and community activities as required.
