Nurse Practitioner - Full Time - Urology Clinic

Shift: 8:00am - 4:30pm

$10k sign on bonus over 2 years. *Position may be eligible for referral bonus.

PATIENT POPULATION:
The patient population served can be all patients including geriatric, adult, adolescent, and pediatric. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

Provides Urology services for individuals presenting for care to Sierra View Medical Center's (SVMC) Urology Clinic in accordance with formal education and clinical training. The participant's skills and talents will be maximized in a progressive care setting with comprehensive technologies and capabilities that advance a full range of urological procedures. Must be able to work closely and collaboratively with Urologists, Medical Assistants, and paraprofessionals in direct patient care, treatment and teaching; assists developing, implementing, and evaluating patient educational material. Contributes to the Sierra View Medical Center Urology Clinic in a positive manner.

Will elicit detailed patient histories; conduct complete advanced practice provider examinations; thorough documentation of the patient assessment and treatment supporting care provided/urological diagnosis identified. Reaches timely diagnosis, orders appropriate testing/resources for patient care needs, develops and discusses plan of care for patient including counseling of the patient and family on health condition; providing appropriate discharge documentation; educational materials; prescriptions; and referrals to specialists, as needed. Engages customers in a patient centered practice, utilizing patients preferred name, being courteous by recognizing and respecting the individual diversity of every patient and the uniqueness of every patient care event.

The role is supported by general policies and standards of practice; SVMC policies, medical staff by-laws, and regulations as defined by Federal, State, and local government, compliance with Medicare/Medi-Cal/CMS requirements of participation, EMTALA, and the Nurse Practitioner Board of California. Provides care appropriate for an outpatient, primary care community health center with a multicultural patient population. Required to participate in the development, review, and approval of clinic policies and procedures as a part of active compliance with SVMC program standards. Must maintain the department in compliance with rules and regulations set forth by governing agencies.

Matrix Reporting Structure: Reports to Manager/Director Outpatient Clinics as delegated by the VP of Professional Services/Physician Recruitment for administration and operational issues. Secondarily reports to the Urologist/Physician with a dotted line report to the VP of Patient Care Services. Oversight of medical practice is by the Urologist/Physician.

EDUCATION/TRAINING/EXPERIENCE:

Minimum education: Bachelor's degree; Minimum Training/Experience: completion of a Nurse Practitioner academic program approved by the California Board of Registered Nursing of California is required; Must have at least 2 years of experience working in an outpatient clinic, experience working in Urology services is preferred. Bilingual Spanish/English preferred.

LICENSURE/CERTIFICATIONS:
Nurse Practitioners must hold an active and unrestricted Nurse Practitioner license and Registered Nurse license from the California Board of Registered Nursing (BRN). Must have and maintain current BLS certification or obtain within orientation process, prior to hands-on patient care. ACLS/PALS is preferred. Must maintain DEA registration and California Nurse Practitioner Furnishing Number if prescribing controlled substances. Must maintain documentation required for credentialing through medical staff services.

Responsibilities and Essential Functions:

1 * Provide all care within scope of license, experience, and training generally provided at an ambulatory/Urology care facility, on an outpatient basis. Including obtaining comprehensive patient histories, performing physical examinations, prescribing medications and therapies within scope of licensure and organizational policy
2 * Develop and implement treatment plans in collaboration with supervising physicians and the healthcare team and provide follow-up care and ongoing disease management for urology patients
3 * Evaluate and diagnose urologic conditions using clinical judgment and appropriate diagnostic testing
4 * Maintain high standard of advanced practice, professionalism, and participate in the training development of the healthcare team. Assist in medical education of both the patients and clinic staff.
5 * Order, interpret, and follow up on laboratory tests, imaging studies, and diagnostic procedures.
6 * Collaborate closely with urologists, nursing staff, medical assistants, clinic leadership, primary care providers, specialists, and hospital services to deliver coordinated care. Supports all providers within clinic and community as appropriate.
7 * Set the standard and role model in your attitude toward patients and staff, thereby, performing duties with respect, dignity, privacy, and modesty towards patients. Must follow AIDET and accepts reassignments and additional duties, when necessary, in a flexible and positive manner.
8 Participate in the development of clinic protocols, as needed. Make recommendations, suggestions, etc., on the E.H.R. and other clinic functions.
9 Fulfill Continuing Education Credit requirements and do all things reasonable, necessary and desirable to maintain, improve his/her professional skills, and renew/maintain his/her active and unrestricted Licensure with the state of California.
10 * Document each on site/in office procedure, prescription, consultation and patient contact in the E.H.R. and on the clinic's encounter forms according to hospital policies. Makes reasonable efforts to see patients in a timely manner and makes every effort to not limit access to care.
11 * Participate in peer review. Report findings to the physician medical director.
12 * Educate patients and families regarding diagnoses, treatment plans, medications, and preventive care. Promote health maintenance and disease prevention strategies.
13 * Facilitate patient referrals and transitions of care. Participate in discharge planning and post-hospital follow-up when appropriate
14 Prepare forms, letters, and examinations on behalf of patients for claims of disability, workmen's compensation, injuries and illnesses, etc.
15 Participate and cooperate in all reasonable efforts with the clinic to apply for and secure public or private grants, contracts, and donations, and all customary and desirable accreditations, licensing and permits.
16 * Participates in meetings and other planning activities for QA/QI of the clinic.
